Gateway - Firefighter Health and Wellness Survey

The "North Carolina Fire Service Health and Wellness Task Force" was formed as an outgrowth of the annual Gateway Conference

One of the top Gateway Conference action items over the past few years has been the need for increasing the awareness of firefighters as to the need for healthy lifestyle habits and maintaining an appropriate fitness level to reduce the likelihood of injuries and health-related problems. 

The Health and Wellness Survey will allow us to establish a baseline by which to measure our future efforts.

Persons completing the survey can simply highlight their answers and save the Word document and send as an attachment to: glenn.clapp@highpointnc.gov  or  the completed survey can be scanned and sent as an attachment to the same e-mail address.
